SIYSL Referee Availablity Form
All referees are required to prepare and submit this form to indicate availablity for assignment. Updates to this form may be submitted at any time as your availablity changes throughoutthe season. |
Referee Game Report
This form is for SIYSL Referees only and should be used to report game scores and to rate teams sportmanship levels. SIYSL Will review all forms and compile a list of ratings. A Sportmanship award will be awarded to the club with the highest ratings. Plese take the time to complete the form. |

Coach's Referee Rating
Coach's referee ratings and comments will be viewed by the board of the SISRA and used to provide better training and direction for our referees. Please take the time to submit reports as needed. |

This form is to be used only by league officials, referees or coaches to report incidents involving games. |
